Home Care Referral Criteria

Trinity Home Health Services provides intermittent, skilled care by Registered Nurses, Physical and Occupational Therapists, Home Health Aides and Medical Social Workers. The goal of our program is to assist the patient to recover, improve or stabilize with the assistance of home health care services.

Patients may need Home Care if they:

• Have frequent readmissions to the hospital.
• Need post-op care, wound care or have a new ostomy.
• Have difficulty with ambulation or activities of daily living.
• Have had an acute flare up of arthritis and wish to increase mobility.
• Have congestive heart failure, chronic obstructive pulmonary disease, diabetes, stroke or other conditions causing frequent trips to a hospital's emergency department.
