Hospice Care Referral Criteria

Trinity Home Health Services provides Hospice Care to help people with life-limiting illness live alert and pain-free, nourished by the support of their loved ones. Hospice staff members are part of an interdisciplinary team including physicians, nurses, social workers, home health aides, counselors, spiritual care providers and volunteers.

Patients may be eligible for Hospice Care if their disease has advanced as follows:

Congestive Heart Failure

• Persistent edema
• Disabling dyspnea at rest
• Multiple hospitalizations

Chronic Obstructive Pulmonary Disease

• Oxygen dependent
• Oxygen saturation less than 88% on room air
• Recurrent infections
• Disabling dyspnea at rest

Cancer

• Widespread, failure to respond to treatment
• Refusing treatment
• Chronic liver disease
• Jaundice, ascites
• Bleeding disorder, elevated PT, not on anticoagulants

Chronic Renal Disease

• Markedly elevated BUN and creatinine
• Urine output < 400 ml/24 hours
• Patient not seeking or is considering stopping dialysis

Dementia, Alzheimer's or Stroke

• Dependent in all ADLs, bedbound
• Garbled speech, weight loss
• Frequent hospitalizations
