Cadillac, MI – Mercy Home Care agencies in Grayling and Cadillac have been identified as National Best Practice Agencies in the National Home Care Quality Improvement / Hospitalization Reduction Project.
Sponsored by the Briggs Corporation and co-sponsored by the National Association for Home Care and Fazzi Associates, the study was developed to identify home care agencies that are successfully reducing unplanned hospitalizations, and to share the information with other home care organizations.

The nationally reported CMS Home Health Compare scores for acute-care hospitalization placed Mercy Home Care in Grayling and Cadillac in the top 10% of all agencies across the country. At Mercy Home Care Cadillac, the percentage of patients who have had to be admitted to a hospital was just 9%, well below the state average of 26% and the national average of 28%. Mercy Home Care Grayling also scored well below the state and national means, having reported just 12% of patients who had to be admitted to a hospital.
