Hospice Care

Hospice is not a place, but rather a way of caring that brings
comfort to people at the end of life, and offers support and
understanding to their families. Hospice helps people with
life-limiting illnesses live their remaining life alert and
pain-free, nourished by the support of their loved ones. Hospice
patients live their final days with peace and dignity in the
comfort of their own homes, or in residences such as nursing
homes.
The Trinity Home Health Services family of hospices cares
for:
- People with an illness causing a limited life expectancy.
- People who choose pain management, care and comfort,
rather than curative treatment.
- People who choose no extreme means to sustain life.
- People of all ages from infants to adults
- People with end-stage conditions including such diseases
as cancer, heart disease, lung disease, AIDS, ALS ( Lou
Gehrig’s disease) Alzheimer’s and kidney failure
- Family members and friends who are grieving the loss
of a loved one.
Holistic Care
Hospice care centers on the many needs of patients and families.
The hospice’s professional staff and the patients’
physicians assist with care by providing patients and families
with:
- pain management
- symptom control
- emotional, psychological, social, and spiritual support
Hospice Staff and Team members
Hospice staff are a part of an interdisciplinary team, which
includes physicians, nurses, social workers, home health aides,
counselors, clergy and volunteers. Other team members include
pharmacists, dietary counselors, physical, occupational and
speech therapists, and other professionals who may enhance
a patient’s quality of life.
Hospice volunteers
The comprehensive services of Hospice would not be possible
without the contribution of our dedicated Hospice volunteers.
Volunteers are the backbone of Hospice care. They provide
a listening ear, companionship, and a variety of other helpful
tasks. Running errands, light housekeeping or childcare are
gifts of time and energy that bring the needed comfort and
rest to Hospice patients and their families.

Hospice Reimbursement
Hospice is reimbursed by Medicare, Medicaid, Blue Cross/Blue
Shield, HMO’s and many private insurance companies.
No one is denied Hospice services based on their insurance
or financial circumstances. Contributions to Hospice, a non-profit
organization, allows us to provide services regardless of
a patient’s ability to pay. Contributions also fully
fund our bereavement programs which offer support to Hospice
families for 13 months following the death of their loved one.
