Coordinating care with the physician, THHS agencies can
provide that important link in the continuum of care between
hospitalization and return to everyday life.
Selection of Staff

The primary nurse assigned to that patient manages all aspects
of a patient’s care. Biweekly care conferences involve
the entire team as a well as the patient and his/ her family.
Certified by Medicare and accredited by the Community Health
Accreditation Program (CHAP), Trinity Home Health Services
consistently meets or exceeds national benchmarks for quality
care as established by accreditation and regulatory agencies.
In addition, as of November 2003, CMS (Centers for Medicare
and Medicaid Services) publishes OBQI (Outcome-Based Quality
Improvement) results on their website. These results were
designed to help the public with comparative information about
home care quality.
Payment Options
Medicare, Medicaid, Blue Cross/ Blue Shield and most other
third party payors will often cover THHS agencies' services
if the following criteria are met:
- Patient is homebound.
- Care is ordered by a physician and is skilled, intermittent,
reasonable and necessary.
